Abstract
Introduction: Health education is a strategy to assist in the care process for patients with chronic diseases, criteria in this way in a continuous and restorative way. Arterial hypertension and diabetes mellitus are prevalent pathologies in primary care, in which these patients routinely use the service, and undergo multiprofessional care on a recurring basis. Objective: To emphasize the importance of health education as a preventive measure for high blood pressure and diabetes. Methodology: Descriptive, exploratory research, based on studies of the literature, from March to April 2018. Some databases were used for a research: SciELO and Google Scholar. From the intersections of the descriptors, articles in Portuguese and English were selected for analysis. Results: Prevention of chronic diseases is a major challenge in primary care, and is directly and indirectly related to health determinants. Hypertension and diabetes mellitus are prevalent in the entire population, and as health education actions in the territory and in the health unit itself, reference is made. It is pertinent to create educational groups that seek to operationalize the care for these patients. Prevention and health promotion are part of the context of basic care, and have a direct impact on the care provided that the diseases and complications caused by it are not affected. Health education offers individuals an awareness of their health, giving them responsibility for their actions, and helping to promote their own care. Final Considerations: Further studies on the topic are necessary to guide health professionals and help them to direct as forms of health education in their territory.
